- Locations: Provider must be local to these locations- Winchester, Staunton, Roanoke, Martinsville, Lynchburg, Harrisonburg, Danville, Culpeper, Christiansburg, Charlottesville, Bluefield, VA
- Board Certified (ANCC, AANP, NCCPA) - REQUIRED
- Active VA license - REQUIRED
