Locum contract- Immediate Housing Response Case Manager
ASAP start 4-6 weeks, $49.07 per hour, Southport, QLD
8:00am-4:00pm, office based, with client visits

What you'll do:

Rapidly engage families sleeping rough; arrange short-term hotel placements
Complete weekly visits, develop specific, time-bound plans, and secure longer-term housing
Use SHIP for intake, case notes, and outcomes tracking
Provide holistic support from intake to exit, including risk/needs assessments and daily living support
Deliver high-quality advocacy and tailored interventions to stabilise housing, health, and safety
Liaise with housing providers, health services, and community partners

Essentials:

Current Driver's Licence (pool car available)
Proven case management across housing/homelessness or related community services
Trauma-informed, person-centred practice and strong stakeholder coordination
Minimum cert III in community services or equivalent.
Blue card and current police check
