We re seeking an experienced Physician Assistant to join a well-established PACE program in Fayetteville, North Carolina, providing comprehensive, patient-centered care to the geriatric population within a collaborative interdisciplinary environment.

What You ll Be Doing

Provide primary medical care in collaboration with PACE physicians
Perform initial, annual, semi-annual, and as-needed participant assessments
Develop individualized care plans within the interdisciplinary team model
Evaluate acute illnesses and manage chronic conditions
Deliver preventive care, screenings, immunizations, and monitoring
Prescribe medications and therapies appropriately
Coordinate specialist referrals and transitions of care
Participate in discharge planning
Educate patients, families, and caregivers
Contribute to staff education and clinical policy development
Maintain accurate and compliant clinical documentation
Participate in on-call rotation responsibilities

Candidate Requirements

Graduate of an accredited PA program
Active North Carolina PA License or willing to obtain
Minimum 2 years PA experience required
Geriatric experience preferred
Experience with frail/elderly patients or willingness to complete PACE training
BLS Certification required
DEA Registration required
Strong clinical and independent decision-making skills
NCCPA Certification required
Clean malpractice/license history highly preferred
NPDB Self Query Report within 30 days required at presentation
