Job Description About the RoleAs a Community Phone Connect Volunteer, you will support DPV Health's vision by assisting the Individualised Care & Social Support team. You will be responsible for conducting scheduled well-being phone calls to over 300 clients in the City of Hume and Whittlesea. This free service aims to provide reassurance and companionship to individuals living alone.
This role is ideal for those passionate about community support and social connection. Your time and dedication will make a meaningful difference in the lives of those at risk of isolation.
Key Responsibilities- Contacting registered participants according to your rostered schedule.
- Arranging shift coverage if unable to fulfill your commitment.
- Conducting weekly well-being calls with assigned clients.
- Accurately recording data in Carelink (training provided).
- Maintaining strict confidentiality and security of client records.
- Following all DPV Health procedures and guidelines.
- Strong listening skills and ability to follow instructions.
- Empathy for older individuals and people with disabilities.
- Experience supporting the ageing community (professionally or personally).
- A genuine desire to help others and promote social well-being.
- Interest in working with individuals experiencing disability, ageing, or mental health challenges.
- Team-oriented mindset with strong interpersonal skills.
- Professional and respectful representation of DPV Health.
- Basic computer proficiency (training on Carelink provided).
- Ability to handle sensitive information with discretion and confidentiality.
- Strong organisational skills and attention to detail.
