Please do not submit candidates that can't cover these dates. MUST have AR license. Rate Caps for Submission:Malpractice Must be included in rates. Orientation 187.50 - half the hourly/callback rate kicks in after 8 hours. Per hour oforientation or training beyond 8 hours On Call Daily 2700 - Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - 4050 - Per 24 hours to include 4 hours gratisCall Back 375 Per hour ofpatient contact hours after gratis hours are completed gratis prorate to MSAterms. Both upper and lower? YESIs laparoscopy required? YesIs any vascular required? Yes- 30 traumas are penetrating sometimes requiring vascular repairs. Annual ER admissions? over 40KWhat level trauma center is the hospital? LEVEL IIWhat are the most common cases the locum will see? Random-A lot of abdominal and colorectal procedures/traumas-Same as above on other procedures For AP:Does the PA/NP need to first assist on cases? NOWill they accept PA AND NP? Would they accept a surgical first assist? NOAverage number of calls per night: 10-20 on average. Mostly for peds patients we will handle Lap Appy's and traumas will get shipped to Children's because we don't have the peds support on it. Deal Breakers?
