Medical Director Post-Acute Care - (FP or EM or IM or Hospitalist, Geriatrician)

Seeking a BE/BC Family Medicine or Internal Medicine or Emergency Medicine or Hospitalist or Geriatrics physician to join a practice providing inpatient primary care to adults in skilled nursing facilities.
Position Details:
• Full-Time or Part-Time
• Flexible schedule. Set your own schedule and the opportunity to build a position that matches your ideal lifestyle and career goals.
• Will not accept 2026 residents/fellows
Responsibilities:
• Primary care rounds, chronic disease management, care planning
• Collaborate with facility staff and patients' families
• Provide leadership, guidance, and clinical oversight
