Long Term Housing Case Manager
LHH Recruitment Solutions is helping our nonprofit client hire a Long Term Housing Case Manager. This position will be 100% onsite and a full-time, contract-to-hire opportunity.
This position will be focused on supporting our client's program participants in their housing journey, helping to meet short and long term goals within this journey. This case manager will be focused on providing supportive services including documentation resources, medical and behavioral health linkages, childcare provider referrals, and other community resources.

Requirements
- Bachelor's Degree required
- Minimum 2 years case management experience
- Must be proficient working within multiple software systems
- Empathetic and compassionate towards others while being able to maintain professional boundaries
- Excellent communication skills, especially in-person interactions
- Strong ability to work well under pressure and de-escalate when needed
- Accuracy and attention to detail
- Can adapt in a fast-paced environment and prioritize tasks
